The State of
New Hampshire REQUIRES that we have a statement on file, prior to your
child's start at camp, signed by your physician and indicating the following:
1. That your child has had a complete physical within
the last 2 years.
2. That your child's immunizations are up to date.
3. The date of your child's most recent tetanus shot.
If your doctor's office does not provide you with such
a statement or record at your child's physical, please download the Universal
Health Form for Camp and bring it to your doctor's office to
be completed.
In addition, doctor's orders must be
provided in order for all prescription medications to
be administered. A permission to administer form must be completed for
all non-prescription medications and can be downloaded
here.
Failure to sign the medical sections of your registration
will cause your application to be returned UNPROCESSED.
